Three Cases of Hospital - and Community-acquired legionnaires'Disease




Abstract
Legionnaires' disease, caused by organisms belonging to the genus Legionella, is an acute lobar pneumonia presenting with multisystem manifestations. The disease occurs as spectacular outbreaks sporadic cases and nosocomial infections. We
experienced
two cases of hospital0acquired legionnaires' disease and a case of community-acquired Legionnaires' disease, diagnosed by bacterial culture and immunofluorescent antibody stain. In the first case of 63 year-old man with intracranial hemorrhage,
L.
pneumophila serogroup 1 was identified by direct immunofluorescent antibody staining of the colony in sputum culture and antibody titer against L. pneumophila serogroup 1 was increased from below 1:8 to 1:256 by indirect immunofluorescent
antibody
test.
In the second case of 39 year-old man with unconsciousness and multiple trauma, L. longbeachae serogroup 2 was detected in the sputum and antibody titer against L. longbeachae serogroup 2 was from below 1:8 to 1:128. In the third case of 67
year-old man
with liver cirrhosis and pneumonia, L. pneumophila serogroup 2 was detected in the sputum and antibody titer against L. pneumophila serogroup 2 was increased from below 1:8 to 1;16. The authors report these cases with the review of literatures.
